Private Work Fees
The NHS provides free health care for most people.
It is important to understand that GPs are self employed and not all of the services we provide are covered by the NHS payments to the practice.
Please expect a fee for non NHS services. The staff or your doctor should inform of any charges before carrying out non NHS work.
We also offer private medicals which may be needed by any agency such as your employer or for overseas visa applications. Please contact the Practice Manager for further details.
We do not provide travel vaccination assessments at the surgery. We provide any immunisations that you are entitled to on the NHS once you have been assessed by a travel clinic.
Please take the form below with you to the travel clinic and get them to fill it in before bringing it back to us, where we can arrange vaccines that are available through the NHS to be made available. WE MUST HAVE THIS FORM AT LEAST 6 WEEKS BEFORE YOU TRAVEL. Any later than this and we cannot guarantee we will be able to provide the vaccinations before you travel.

Self Referral To our Midwives

Please print, complete and bring this form with you when you attend your appointment.
The community midwives offer advice on all aspects of antenatal and postnatal care. They hold regular clinics at the surgery. You will be referred to them by your doctor.

We run a number of weekly clinics for patients who suffer from chronic illnesses. These clinics are run by the practice nurses who have specialist training in these areas. You will be invited to attend one of these clinics if you suffer from a chronic illness.
